Side question... if this has been answered elsewhere please just point me to it.
I autotuned USTVNOW with all 30 stations, but I only have the basic. I turned on Stream Validation, and I figured that would weed those channels out since there is no stream for them, but it didn't.
Is there a step I am missing?

Leave it on low for now... Low verifies the plugin is installed, High checks to make sure the plugin link is also valid.
Its currently to buggy for casual use.
Edit 2: Tried high validation, didn't work. It just removed all plugin related channels.

I am not sure what log to show since my problem isn't really an error. Essentially, my EPG is showing 30 min shows as an hour; it isn't doing it for all of them, but just some and it is seemingly random. All content is .strms.
Oh, and a side question, is there anyway to increase the media limit on Youtube channels? The channel I want has far more than 1000 vids, so it is seemingly just pulling the first 1000 then shuffling those rather than taking 1000 random videos.

As for youtube, 1000 is a reasonable limit. You can try splitting your channel into playlists using one of the many youtube playlist tools available online.

I was doing it using TV Genre channels with rules, but I have switched to Custom Playlist channels and I am having the same issue. I have tried other .strm files from various add ons and I am still getting incorrect guide times.

Think I found a small bug, When adding a channel through the configuration tool of these types: TV Network, TV Show, and playlist, then navigating to advanced channel rules to set the channel name. Once it is saved. It puts a leading space in the settings2 file for the channel name setting. I think this is why some users are reporting that their logos aren't showing up. Thanks. This was a great find, I went through my settings2.xml file and every renamed channel had a space in front after the double quote. I removed the space and all the channel logos populated correctly. Thanks for posting the quick fix.

Hmmm this is strange because your issue is bypassing all the norms.
1. Strms don't have a duration, unless its been scrapped by kodi or it has a matching nfo with duration info.
2. If no duration is found PTVL should default to 60mins not 30.
3. Playon never returns accurate runtimes, so PTVL uses a lot of guess work.
You will have to use the info above to trouble shoot...

The .strms have been added to the library, so they should have that info. Is there a way I could make PTVL default to 30 mins, I'd settle for that.

I seem to have and issue with 0.5.8d. My screensaver never kicks in any more, nor does the system shutdown after 1 hour of being idle. Had developer fritcsh look at the logs and we removed PTVL and then my screensaver started working again. I think something in PTVL is preventing kodi from going idle.

Edit: I have removed, reinstalled, restarted, check for updates, force refresh and every variation I can think of and every time the log says "returned no addons, listing may have failed".

Which flavor of kodi are you running?
Experimenting with this in win7 x64 (as I know OpenELEC will be an another hurdle ).
Only just realized I am using 14.0. Upgrading to 14.1 and starting again.
Update: I dug a bit deeper. The repo zip points to "http://raw.github.com/Lunatixz/XBMC_Addons/master/zips" which redirects you to "https://raw.githubusercontent.com/Lunatixz/XBMC_Addons/master/zips" and promptly dumps you to a "Not Found" error.
Well I tried everything I can, does anyone know how to force ptvl to hide channels that have no content? I.E. USTVNOW stations that are no longer subscribed to.

I cant find my settings2 xml file on a mac so I just put a space before the file name for the appropriate logos. Worked like a charm!
