I am having some difficulty trying to figure out if you can run nextpvr with an OTA signal while at the same time having access to an IPTV account. It seems I can only do one or the other not both. Is there any way to do both together?
NextPVR doesn't do IPTV (or at least not the internet based channels you're probably meaning). It's only for capturing from a physical tuner or video capture device.
You can do OTA and Cable, but IPTV seems to be elusive. I've done it, but it's a pain to maintain and you must use IPTV Stalker client & NextPVR not IPTV Simple client. The NextPVR backend no longer supports IPTV (believe me I tried), it pulls in the channel list and then just stops.

The OTA/Cable channels show up everywhere in the list using IPTV Stalker. Therefore you must use SQL to modify the Channel database. But as soon as another channel is added to IPTV, your list is gone.I eventually gave up.

What would be better, IMO, is the ability to switch between one PVR client and the other and then reload the db after the active PVR becomes enabled. Or even another profile with different clients, again I tried but was unsuccessful.

If you use one of the guide addons you can add the NPVR live channels as a favorites. Use webgrab+plus for the XMLTV source and the channel are in the guide, at least the one I'm using at the moment: http://forum.kodi.tv/showthread.php?tid=282157
If you are looking for a full featured Set Top box experience I'd suggest PseudoTV Live http://forum.kodi.tv/forumdisplay.php?fid=231
