I'm currently thinking that setting the external player to point to a script, which launches TMT5, then clears XBMC's playlist via http-api or similar could be a way to go... but it seems a little messy.
The script may be a little messy, but without any insight from the devs or other knowledgeable users we don't have many other options.
I've come up with a workable solution ... it's not as elegant as I'd like, but it allows me to replicate the "Play Disc" function and launch the TMT media center version for DVD, Blu-Ray, or HD DVD discs. I'm relying on my frail memory to recreate this process, so please forgive any omissions.
First I downloaded and enabled the Executor program add-on, which allows the user to launch any external program they want.
Next I discovered you want to create a .bat file in your root c:\ drive that points to the executable. And it needs to be in the old-style DOS (short filenames) format. Here's how mine looks:

(I forget why I went with the .bat file. I think because the player didn't go back to XBMC when I exited TMT3.)
Meanwhile back in XBMC, go to Programs and then Executor. In the context menu (right-click) choose "Add Program", call the new process "Play Disc" and navigate to the tmt3.bat file. Before you leave Executor be sure to add this new creation to your Favourites.
Lastly you need to go to Skin Settings -> Favourites Shortcuts and enable one of the desired favourite buttons. Choose your newly created 'Play Disc' item, and it should now appear on the home menu bar.
This method has a couple of drawbacks... for one, it flashes the command prompt window and desktop for a split second. For another, unlike the XBMC's built-in "Play Disc" function, it shows up whether you have a disc in or not.
Of course, this means you have two options for 'Play Disc'. I disabled the built-in 'Play Disc' function by editing the home.xml file (found in the 720p folder) to delete all references to the Play Disc function on the home screen.
