OK after a bit of a cry everything's working OK now!
So for anyone stumbling across this thread: I used the settings in the table on this site to get HDMI audio working: http://wiki.xbmc.org/index.php?title=HOW...,_or_GT240
So, in XBMC I set "audio output" = HDMI, and set "Audio output device" and "Passthrough output device" both to "plughw:0,7". I also had to reboot to get sound working. (plus make sure you follow the stuff about ALSA, and unmute the four S/PDIF outputs by pressing 'm' and navigating with arrow keys)
