My guess is your ATV2 is struggling with down mixing the 6 channel audio to 2 channel stereo.. Maybe you could give it a try with passthrough enabled?

I have similar behaviour. After I have started XBMC playback is jerky but I can fix that by restarting XBMC.

Look closer - its in system->system->audio

i have the audio settings, but can't change the output device - the only option is Default
