Not sure what changed on my XBMC setup, but if I select Random Movies (fan art) for my MOVIES background, it only shows images of unwatched movies in my collection. Before it used to randomly show from all movies, not just unwatched.

Choose Background > Random Movies or Live DVD cases of random movies

In both cases above, it only shows unwatched.

I figured out the issue. It was caused by the Skin Widget.
Go to Settings > Addons > Services > Skin Widget > Configure
Turn off 'Only unplayed & unfinished' on both Random Items and Recent Items
Save and exit XBMC.
Next time you open XBMC, it should now show all fanart/DVD cases and not just unwatched.
