With a deafult installation on windows XP I get the video going fast and slow to try and keep syncing with audio that is playing fine. So its like slow motion followed by rapid video.

However, if I display the main menu overlay on video then everything is good. How can I fix this?

Check the Vertical Blank sync option. Try changing it.

Of course, that's just a guess. If you can supply info about your setup, as well as a debug log, it would help others with what is going on in the background on your system.
