I would like to see play history for TV show or movie, but I haven't been able to find a plugin that does this. Does one actually exist?

I don't know such plugin but you can add two smart playlists. One for movie and one for episodes. Content is e.g. played the last 30 days and order by last played sort order descending.
Add the smart playlists to the menu and you have the solution. I recently tried this with the skin Aeon Madnox but i'm sure it's possible with other skins too.
