I'm running XBMC on my laptop which looks great at the laptop's 16:10 ratio. But I also connect the laptop to a regular 26" 4:3 CRT tv via svideo. The tv-out display is at 1024x768 and both default skins look terrible. All the menus and fonts look like they have been squashed in left and right. Videos show fine but the menus are not the right shape.

Am I doing something wrong or do these skins not support a 4:3 display ratio? Are there any skins that really look good on an older CRT tv?

Try Project Mayhem III (not PM3.HD), xTV, and MC360.

For classic 4:3 TV screens also try this skin: "Connected"

Its simple and customizable.

I personally also use XBMC (Windows) to CRT TV 4:3, and I am currently using skin PMIII-HD, but I modified font.xml file for bigger fonts in most areas of the skin. Result is nice, except proportions of pictures in some menus view types are slightly distorted.
