So i just tried to play mp4 of Resident Evil.
Hit play no audio, hit back to exit no XBMC audio, exit XBMC no atv audio?

What version of the ATV OS are you running. I had this issue as well. When I checked I had the ability to update to ver. 3.02. Once doing that and redoing the patchstick, my audio problems were fixed.

ATV OS is 3.0.2 already and latest XBMC

has there been a fix for this yet? I'm having the same issue

I reinstalled - changed my audio output settings and make sure all your resolutions are set to 720p then it worked.
