I used AnyDVD to rip an image and that will show DD 5.1 on XBMC amd Dolby Digital on my receiver (pioneer VSX-32), I used HB and make an MKV with 5.1 passthru and that shows DD 5.1 on XBMC and DD on my receiver, but when I used HB and made an M4V with 5.1 passthru I get DD 2.0 displaying in XBMC and simply STEREO on my receiver.

I'm only including one audio source in my movie.

I've read a few posts, but I haven't seen any answer to this.

My work around was to use AnyDVD to rip movie fold to my desktop, then use DVD Shrink to cut out everything but the movie and rip it to a VIDEO_TS folder. I know the end result is a file much larger, but heck HDDs are way cheap these days.
