I have 4 XBMC boxes, I want to add a TV card in 1 box that supports 4 streams.

My question, Will I have Live TV on all 4 boxes or just the 1?

All four. If you put a suitable backend application onto the one with the tuner card, you can stream that TV signal over your local network to any of the clients. However, one tuner is unlikely to allow you to watch multiple channels at one time (you can if you're lucky, but they need to be on the same mux - the same carrier frequency).

I have one server with four tuners, and feed that to any combination of about half-a-dozen XBMC installations.

Sorry, missed the "supports four streams" bit of your post. If that means you have a card with four separate on-board tuners then yes, you could theoretically watch four different channels at the same time - host system CPU and bandwidth allowing.
