How can I configure, e.g. in Confluence, the main menu including the all of the sub items, so that it might look like this:

You'll need to use a different skin. The one in your screen shot looks a bit like Aeon Nox.

Yes, it is Nox. Very nice skin, but I would like one that is easier to update (actually automatically) / handle. After the update to Gotham I have to do all of my settings again. And I thought a / the default skin updates automatically and would keep my settings after a next update.

From frodo to gotham, aeon nox changed quite a bit...so it didnt keep settings. Confluence might keep the settings, but is substantially less customizable. Choice is yours.
