This has happened last week with The Strain, where I had 10 episodes, then added the 11th, updated and it wouldn't show up in the library (scan for new content, etc. no go).
Now it's occurred again with Penny Dreadful, I added one episode fine, now I have added three more and they do not update during library update. I turned on debug log and here is what I copied from the time I turned it on to around when I tried update library: http://pastebin.com/9h7gA5YL
