Can anyone post a sample file?
Where should it be stored?

You can create and store favourites files with the DMS channel list editor. See

*local* favourites files should be stored on the device that runs Kodi. You need to copy it from the DMS directory.
The idea of the *local* file is to have different favourites on different clients/users.

Unfortunately, *local* is a rather vague location. Where does the add-on expect it to find?
