Additional info is not showing for me even when it's enabled in the settings, will it only work in Nexus?
It doesn't matter if you use Matrix or Nexus. In movie widgets it isn't possible showing additional info underneath items because these widget use poster view. For TV show widget you have to
disable the setting above.

Disappointing. I've been playing around with the option on & off. I heavily use Favorites and names under favorites thumbnails gets disabled with the option on. No way to change that either?
No. Not for Poster views.
I've added some modifications to poster view (this affects the "music videos" widget too):
3.4.2+matrix.10 / 4.0.2+Nexus.10
- Widget header for TV timer in Home menu modified (Nexus only)
- include 'Jump to Letter' in Flix view
- make 'Play Trailer' in context menu customizable
- show next timer and available disk space in tv timers window
- remove skin setting 'use poster view for music videos'
- add title labels to poster view widgets

Does this actually stop the image from trying to load? I'm using a firestick 4k max and have thumbnails/images on an external USB drive so they load in a bit slower and was wondering if the mild stuttering I have when scrolling through my TV library view is due to the background trying to pull in the image of the currently selected show, even though I keep opacity at 0. I didn't notice that stutter using the default skin.

No, it simply overlays a mask with a various opacity. The images are still there. Second, this Mod uses some more of CPU/GPU resources than the default skin - so it's possible that a firestick is on the egde with this skin.
Would it be possible to implement a setting to disable the background images entirely?
I could combine this with the setting for showing animated backgrounds on home widgets (except for the fanart view - here it doesn't make sense to disable fanart).

In cases when there are no available PVR artwork "e.g. live events", would it be possible to default to the channel logo instead of the current default icon in both EPG and OSD? I think it would look better.

The channel logo is the default fallback if artwork isn't available (PVR artwork module or EPG). Maybe I misunderstood that? Could you provide a screenshot?
I had only one channel where the channel icon is disappearing (even if it exists) so I could check out this behaviour. For some reason the PVR artwork and the EPG event icon signaling a picture but doesn't provide a filename. This has been fixed and will be available with the next update.

Edit: Just saw that the false media flag is shown when the regional date format is using AM/PM. Has been fixed too.
Yeah, I remember channel logo used to show not too long time ago, could it be due to another add-on or skin? I have enabled the online scraper under PVR Artwork Module, can it cause this issue?
