I'm having this issue, when in media preview some thumbs dont show unless i put focus on them, other dont show at all. 
All my thumbs are in the movies folders and where created with export library.
I'm using mediastream v1.0 with t3ch's build dated 25-01-2009.
In list view every thumb show, i have fanart hidden.
EDIT: 2-3MB of free RAM in media preview, that seems to be the problem for thumbs not showing.
