XBMC stores these images only in their own cache.
When you export the database a hidden folder in the movie folder will be created.

If we save the thumbs in the hidden folder will XBMC use them or is a waste of time and space?

Ember allready support downloading of actor-thumbs. And yes, XBMC use this images instead of links in NFO's.
XBMC create thumbs and save it to the normal cache like all other images.
The scraping/db-build in XBMC is mutch faster with pre-downloaded actor images.
But Ember don't support actor images in tv shows (is is commented out).
