Sorry, I don't like NVidia's attitude towards open source and recommend using alternatives.

 You are using crap from NVidia, right? I don't think NVIdia has fixed the 23.976 bug (all plattforms affected). NVidia crap was known not to switch to 23.976 but 23.971. Search the web for this bug or get yourself some decent hardware.  
 How did we get onto blaming Nvidia for this?  I don't get it.  I can use Nvidia hardware acceleration in Kodi 17.6 and MPC-HC/MadVR at 24Hz without issue, only Kodi 18 has this problem. 
I mostly blame NVIdia for their attitude towards open source. They don't support open source, I don't support NVidia. I am not saying that this issue is not fixable on Kodi side. I am saying that I don't care and recommend not to by NVIdia for usage with Kodi. There are better options.

ok i'll got it.
A few months back i was thinking of upgrading my two HTPCs with a cheap/decent PCIe passive cooled GPU which could handle 4K.
My decision was the Nvidia 1030, which now i feel was the wrong one.

Will try to sell it and get another one.
@FernetMenta can you please give an recommendation?

I was looking for a cheap passive cooled 4k capable radeon for windows, but not found one (the R5 230 sadly is not 4k capable).

PS.: Dont want to swap out all my hardware (i3-3220, ram, mobo...)

As i said, i can borrow someone with dxva knowledge my 1030 for some time.

Please dont abaddon nvidia support in kodi, ill think there are other users out there which are using their products.

 Don't get this wrong. This is my personal opinion not an official strategy by team Kodi. Everybody is welcome to continue suppport for NVidia harrdware or even implement their new proprietary decoding API on Linux.

As I'll said, I'll could send and borrow you my 1030, if hardware is the key to solve the issue.

Tried both the FPS_test_1080p24_L4.1.mkv and FPS_test_1080p23.976_L4.1.mkv files with KodiSetup-20180529-795b10a6-master-x64.exe, and did not experience any playback issues. 

I am running Windows 7 x64, an MSI R9 270, and i5-4690k CPU

Which gpu do you have?

Do you use change refresh rate?
If not, try to activate it, Whitelist all refresh rates at 1080 resolution and try again.

I have a MSI Radeon R9 270 Video Card that is OC to 1100mhz GPU/1450mhz Memory

No, I dont use change refresh rate. I keep Kodi at 1920x1080p with Sync to display set to ON. I also have 1920x1080p 60hz and 1920x1080p 59.67hz whitelisted.

I'm having this same issue running 17.6 on my 9 year old Win 10 machine which has a newish GT 710 GPU.

I've found the following results on changing various settings playing a 1080p x264 video:

DXVA On, Refresh Rate Change On, Sync Display On - Continuous stuttering, CPU ~30%
DXVA On, Refresh Rate Change On, Sync Display Off - Frequent stuttering, CPU ~15% -> No stuttering with overlay
DXVA On, Refresh Rate Change Off, Sync Display On - No stuttering, CPU ~30% BUT some noticeable audio clipping with bitstreaming to AVR
DXVA Off, Refresh Rate Change On, Sync Display Off - No stuttering, CPU ~30%, audio fine

I've stuck with the final option (software decoding ) as it gives the best stability albeit with a higher CPU load.

It's a frustrating bug given the overlay "fixes" the issue. Has anyone worked out how activating the overlay can possibly be influencing things?

Just use v18 nightly ...
That doesn't solve it for me.
Just disable Dxva, enable sync to display and disable change refresh rate.

That's the only reliable workaround for me.

Not tried yet, but a good idea. Do you use an older driver which you can recommend?
