After finally getting everything up and happy, I got the urge to add a DVD-ROM to my box, power down, plug it in, bios sees it and... XBMC dosen't...

Is a complete re-install necessary

I'm not sure what you mean. In the top corner, next to shutdown icon, do you have the "disc" symbol?
Have you inserted a DVD without anything happening?
Are you running xbmc-live, xbmc on top of a linux distro or something else?
I believe DVD-players are plug'n'play.
