24p lipsync issue after pausing

Hi,

Running:
Windows 7 64-bit, 4gig RAM, Intel C2D 2.5Ghz
GPU: ATI HD 5450
ATI CCC Driver: 13.1
XBMC Frodo 12.1 (issue also on 12.0)

Connectivity:
HTPC GPU 5450 ---HDMI---> AVR YAMAHA DSP-Z7 -----HDMI-----> TV PIONEER KURO 8G


The Issue:
Been struggling with this issue for a long time, but today put some time in to try an narrow down where the issue is.

When I play some 24p video content with my TV also set to 24p (23.976) I get lipsync issues (Audio ahead of video - about 100-200ms).
When the issue occurs it's present from beginning of playback and after pausing the video for any length of time.
The issue can be fixed simply by skipping back 30 seconds, but will reoccur the next time I pause or restart playback.

After trial and error with XBMC settings I have found the following:

Doing either of the following FIXES the issue:
- Disabling "Vertical blank sync" FIXES THE ISSUE
- Playing 24p Content while TV is set to higher Refresh Rate (e.g. 50, 60) FIXES THE ISSUE

Obviously the above fixes do not help due to tearing and judder being introduced instead of lipsync.

Doing the following makes no difference
Dxva2 disabled and software decoding enabled.
Bitstreaming disabled or enabled.
Sync audio/video enabled with all possible settings
Using DirectSound or WASAPI

I have already been through the audio troubleshooting guide for Frodo.

Any ideas? Does the fact that disabling Vertical Blank Sync fixes the issue point to any particular component.
