If you have done your own custom screen, you may have not set it in your WindowXML file? (The default file coming with VideoExtras should display the tick OK). I don't know what else could be stopping the watched state flag (Tick) from showing.
The Tick (or any icon you prefer for the the same purpose) uses the default XBMC image: OverlayWatched.png http://wiki.xbmc.org/index.php?title=Default_Icons
Probably because I dont have a OverlayWatched.png and OverlayUnwatched.png in the root of the media directory. Seeing as this xml is provided by the addon, the addon should include those. If the addon depends on skin textures it becomes some mix of textures and will lokk different on each installation, which is probably not the way its intended.
