is there a default volume amplification i can set so that i don't to set it each time for a channel?
+15.0 dB seems the best for me as most channels have very low sound

In the OSD for audio you should be able to increase the amplification, then save for default in all videos.

In my Audio OSD, the Volume, Volume Amplification and Audio Stream adjustments are greyed out and unavailable to me. It was like this in Helix and now in Isengard. Am I doing something wrong?

You can't adjust volume of passthrough formats (as they wouldn't then be passthrough) so you were probably playing something containing Dolby Digital, DTS or one of the other passthrough formats. To adjust volume for files containing passthrough formats the audio must be decoded to LPCM, to do this you must disable passthrough in Kodi.
