A full, complete, unedited, unadulterated and in a pastebin
Debug Log is required for any problem. (except for passwords etc)
Its not up to who asks for help to decide what information the helper needs to see or not see in order to provide better assistance.
Within reason.
In the meanwhile you may want to add a little line to your advancedsettings.xml and see how that goes.

and the reproduce with debug log like so , enable debugging in Kodi, restart kodi, duplicate issue and pastebin the resulting log. Should be fine for pastebin like so.

Well dont use path substitution for thumbnails is one issue, doesnt work properly and is not supported properly. See BIG warning at page

removed thumbnail path substitutions from all kodis in the house and its still happening.

sounds crazy, but turned off my ATV2 kodi and it hasnt happened since.
will test more and post if the problem occurs again.

Doesnt sound crazy at all... You probably had two machines on the network triggering library updates at same time or fairly close to each other, hence hashes were changing and you experience that, would be my guess.
In my opinion only one box in network should act as master for triggering library updates so that the databases and content are managed in a more sane manner. But never used mysql with Kodi so wouldn't know whats best or whats ideal.
In any case dont path substitute thumbnails, bad idea with current state of affairs.
