Thanks for the sample. This is actually caused by out-of-spec lame encoder. Not even QuickTimePlayer is able to play this. Nevertheless I created a ticket on ffmpeg: https://trac.ffmpeg.org/ticket/5689
Maybe they implement a workaround.
