Does Kodi upgrade actually update advancedsettings.xml?

I'm curious what else I might need to change?
(The two I've highlighted are now of course "cache" and "memorysize" - I only found this accidentally)

No. Kodi never touches advancedsettings.xml other than to read it at startup or on profile load.
You need to make any changes (eg cache settings) manually.
Edit: fwiw I can't see anything in the above other than cachemembuffersize that's changed.

So I'm guessing that I would have needed to pay close attention to the release notes to notice this one basically?

I upgraded and found issues from 15.2 -> 17.3

I guess so. The wiki advancedsettings entry will usually have any changes, or sticking 'advancedsettings.xml' in the search box here should flag them up.

Note changes section 3

In Kodi v17, the cache-related tags were removed from <network> and placed under a new <cache> tag. In addition, the following tags were renamed:

<cachemembuffersize> was renamed to <memorysize>
<readbufferfactor> is renamed to <readfactor>
