Solved Frame Drops, Skippied Frames With 720p content Upscaled to 1080p

Since upgrading my video card I am now getting frame drops on content that is in 720p and being upscaled to 1080p 24p

The following steps will "correct" it but none of the options are feasible or solutions

1. Turning off hardware acceleration - This is not feasible as I have UHD content
2. Turning off "Adjust display refresh rate" - Again, I need my content to play in 24p
3. Turning on "Sync playback to display" - This turns off audio passthrough
4. Changing the resolution to 1280x720p - Content will playback at 1280x720p however, my tv won't adjust it's refresh rate to 24p and will remain at 60p.  
                                                          - The GUI also looks terrible on 65 inch tv

 weird. i spent hours trying different things and i've had no luck. 

One thing I have noticed is that the frame drops are very noticeable on the first run. If i stop and replay the same video file it's barely noticeable and drops a lot less frames. Could it be a buffering issue?

What should be my settings in Nvidia Control Panel? I'm almost certain I have everything off.
