I've been using an old version of Media Companion, one of the first 2.xxx (january release) releases and it has been working great, but last week XBMC stopped picking up the .nfo files for some odd reason.
So I've installed latest SVN build this weekend and the newest Media Companion, but I'm still having the same problem. The earlier nfo's scraped by Media Companion 2-3 weeks ago works great.

The only difference is <playcount> and <Rating> I think, but dollhouse.1x07.nfo is not picked up by XBMC. But this problem started with XBMC "Atlantis" release and the old Media Companion, so maybe it's theTvDB's fault? But I have no clue, it's just annoying that it doesn't work all of the sudden
It's not the fault of TVDB, the information has been downloaded correctly.
The only thing that should cause this is if the filenames of the media and nfo file are not the same. As a matter of curiosity, could you edit the file with the ratings tag and move the tag below the episode and season info. i.e.

XBMC needs the episode and season tags and may be looking for them after the title (I really doubt it though)
This might be best brought up in a XBMC bugs thread. I know that there has been some discussion about whether playcount should be imported or not but in any case, the nfo is still valid and shold work fine.
