In the past week, I have found that two shows have not been added to the Kodi TV Shows Libarary.

The two files in question are:
/storage/dvr/The Simpsons (1989)/Season 31/The Simpsons (1989) - S31E10 - Gorillas on the Mast.ts (Added 11/3)

I can still view the files through the Video -> Files menu.

I enabled DEBUG logging a few times during the past week, and performed manual library updates at that time to try to capture why those two files were not being recognized.  All other new files in the "TV Shows" library are being properly recognized and added.

I think I just answered my own question.  This is not a KODI issue, but a DVR/EPG issue.  The DVR did not properly name the files, thus the scrapers are not able to obtain the correct information.

Should have been named:
The Simpsons (1989) - S31E05 - Gorillas on the Mast.ts

/storage/dvr/Modern Family (2009)/Season 11/Modern Family (2009) - S11E10 - A Game of Chicken.ts (Added 11/6)
Should have been named:
Modern Family (2009) - S11E06 - A Game of Chicken.ts
