Which I fully agree, hence I still think renaming the movie is best. Casino Royale becomes "James Bond 21 - Casino Royale". Such a system works for all movie types. And the original title is still in <origtitle> right? so nothing is actually lost.
In fact, I use tagline to add on the foreign language name of a film and the movie title for the english version and lock tagline as well.

The only downside is that your list would look all weird because you could have some movies stuck in other letters (Like if you set sort title for "Casino Royale" to "James Bond 21"... it would show up in the J's instead of the C's).
