If you really want to keep your folder structure, you can activate the "recursive scan" option of Advanced Launcher and name your files (items, thumbs and fanarts) with the name of the game.

By default these images are saved into the same directory of the executable files... but if the XBMC user you do not have write access to this directory... you will have a error. That's a supposition, until I cannot take a look at your launchers.xml and xbmc.log files.
