DXVA ON (if OFF everything works ok)

The Intel video chip has been known to give problems with hardware acceleration. If that's the cause it would explain why you get problems with acceleration enabled but it works OK when DXVA is off.

If the problem is the intel chip, not much anyone here can do about it.
