Solved Strange but serious music playback issue

I have XBMC set up on my Arch 64bit machine.
I am getting a serious error when trying to playback any music from my library, well 2 issues really.

Firstly any time I play a track it always plays 2 files, one track that I wanted and another (either randomly if selecting one track, or from the playlist if listening to playlist).
If I pause, the random track pauses but the selected track continues playing. After this I cannot resume any control of XBMC.

Secondly, the more serious issue, after a few minutes of listening to a music track my whole system 'whites out' (hangs with no audio, and white/black/grey display). From this I have to force a hard power off by switching of the power supply - not good!

Does anybody have any ideas?
If you need any more information like trying error logs or system details let me know.

Threat over, I managed to get everything working again by simply flushing my library database and re-indexing the whole Music library.
Everything seems to be working fine (so far)
