Solved Failed to open image

Just upgraded from 14.1 to 14.2 - backed up my Thumbnails folder and rebuild with lastest OSMC/Kodi - recopied Thumbails back.

I'm having an issue now where none of the thumbnails are loading for the series.

In the logs I am seeing this:

could be your Textures13.db
or you could try to check Texture_Cache_Maintenance_utility (wiki)

Dude your a genius!

Copied over my backup Textures13.db and its working 100%

You learn something new everyday. I use a remote mysql data and always thought this db was kept in the mysql db.

Need to make sure I keep backing this file up!
