I searched and could not find any topics on this. I installed the NBC Sports Live Extra add on and it's working fine until I select the premium content (mainly to watch EPL games). Is there a fix for this or do I actually need a cable provider login?

Your question is not really PVR related since you're talking about a generic video addon. You need to ask whoever created the addon.
