You don't appear to play anything while taking the log, to be of any use we need to see a 4K file being played.

T95ZPLUS is a S912 afaict.

There are plenty of boxes with this SoC, but not yet a decent Android firmware, afaik
In this case, mediacodec cannot be instanciated, so it falls back to s/w decoding/rendering, which doesn't support 4K on those devices.
If only there was some type of "forum" with warnings about buying crappy S912 boxes out there, it would be a magical place full of unicorns and lollypops.... or just a couple links away http://forum.kodi.tv/showthread.php?tid=252916
