So it turns out my problem was indeed permissions based. For some odd reason, folders would not inherit permissions correctly. My new folders would show up and everything would show up inside (.nfos and pictures) but no movie file. To fix, went to root and redid permissions, ensuring subfolders inherited permissions from parent... giving read/write to Everyone. Worked like a charm. All newly created files show up now, no issue.
