@daffeh: No, XBMC requires TV shows to be sorted into folders for them to be added to the library. That is independent of the scraper you choose. See Video library/Naming files/TV shows (wiki) for details.
@kevn57: There's only one setting that really affects fanart downloading, which is "Enable fanarts" located in the "theTVDB" tab of the scraper settings (the other setting, "Continue lookup if no fanart found", rarely applies anyway). If you have managed to disable that, make sure you re-enable it on the source folder (highlight the source folder, bring up the context menu, select "Change content", and then "Settings"). Just changing it via System->Enabled Add-ons->AniDB.net [MOD]->Configure won't change it for existing sources.
If it's not that, I'll need to see a debug log (wiki) of you refreshing a show.
