It seems that version of Kodi Krypton is extremely unstable based on its log file. In either case, that's neither here nor there.
There are two reasons why From Ashes failed to install and/or work. The first is indicated by your log file during the installation of the NextUp Service:

The second reason which is unrelated to you but my fault entirely is that I did not include a:

DialogConfirm.xmlWhich replaces several dialogs. As a result, you'll run into issues such as the skin not loading.
The update in 6 days ("Summer 2016 Update" - June 18th) will have full support for Krypton although still experimental.
The only reason 4.0.01 wasn't pulled was because others realized they could just copy a "DialogConfirm.xml" into the skin's 16x9 folder and it would load up flawlessly.
