So I guess another example of this "custom" dialog is: System-Music-Clean Library-Yes - No, back/space doesn't work I need to use esc/home button, but at least after I exit the dialog the esc/home should take me to main menu instead it acts the same as back/space going back by one level.

yeah every skin seems to have at least one dialog you can't get out of. Its definitely necessary to have 2 separate buttons at the moment, which just gets weird because remotes usually only have 1.

avs preset and avs plugin selection in music fullscreen i mentioned before are another examples.

ever considered creating bugreports? back should get you out of all dialogs, if it doesn't there is a bug

I posted patch that allow ACTION_PARENT_DIR (aka backspace) to close dialogs (at least those that ronie, Japaja and evlo mentioned here).

Also there https://github.com/xbmc/xbmc/tree/master/system/keymaps seams to take care of some of the flaws for remotes. Interestingly enough i did not found any premade keymaps included in my xbmc installation also i dunno how to use MCE as remote in xbmc without mce remote plugin, but only with keymaps and same goes for using harmony as joystick.
