Finally figured it out along with using a powered USB hub for the CEC adapter.
In autostart.sh I have added
/storage/.config/hdmi-fix.sh being:
There's probably a better way to do this (and I'd love to know!) but it seems to work for me from a cold boot with TV/AVR off.
