Check to see if you are actually getting any dropped or repeated frames. The custom mode screws with madVR's frame drop/repeat estimate.
And yes, you can just switch to 23hz and change the bit depth, save, and then return to the desktop refresh rate. I recommended playing a video as people don't always know which refresh rate to choose in the control panel. Playing a video can sometimes be easier.
