madvr has a bug IMHO with refresh rate switching, when I use it I get HDMI loss and black screens stopping 3D movies when it has to switch back from 3D 1080p to 4k 60hz desktop or KODI DS GUI. If I set my desktop to 1080p before I start the 3D movie its fine as it only has to drop back to 1080p. It only happens with MADVR refresh rate switching.

I now use KODI DS and MPC refresh rate switching, it does the job for me.

I asked MADSHI to look into it but he refused citing driver issue even though I its only an issue with MADVR, so we're stuck with it, at least on AMD RX cards.
