Strangeness generally appears when you start copy/delete/replace images out of XBMC or Advanced Launcher. In that case, XBMC cache is not been updated and the cached images do not correspond to the images present on your hard drives. You generally have to wait a couple of days to have the XBMC cache automatically refreshed.
Additionally, I'm getting some strangeness with scraping art and game data. Is there a way to clear caches or any DB, so I can scrape again and know everything is clear.
As far as I know, there is no option into XBMC to force to refresh the cache.
To avoid these cache problems, I strongly suggest you to manage your images only from Advanced Launcher and copy/delete/replace images on your hard drive only when they are not used anymore by XBMC or Advanced Launcher.
