Windows? You can use MKVMERGE:

If you wanted xbmc to see them as one film you don't need to merge them, use stacking:

(in case it saves effort of combining them)

cover:
movie.tbn or movie-cd1.tbn

and backdrop

movie-fanart.jp or movie-cd-fanart.jpg
