Under System/Video/Playback, set:-

Render method - autodetect
Adjust display refresh rate to match video - enabled
Sync playback to display - enabled
A/V sync method - Video clock (resample audio)
Allowed error - None

Under System/System/Video, set:-

Vertical Blank sync - Always enabled

.....

I had problems with a slight stutter every 4/5 seconds which I put down to a software attempt to resync the audio and video. The A/V sync method above seemed to eliminate the problem.
