Uncheck use local files and that should add it to the database without saving a copy to your folders.
Thanks for the help, that worked like a charm! Although I think I now have to clear my database because its 3.8gb bigger than it used to be before fiddling with artwork downloader lol.

Edit2: It looks as if, in a moment of confusion, something has attempted to assign my monster hunter ISO as background art, thus renaming it and copying it to the thumbnail folder. Cheeky.
