The issue is simple : Artwork downloader does not load anything. If I run it directly, the script downloads things. Of course, the idea is to run that script 'as a service' with an automatic run and a background downloading. I read the documentation and found mention of the need of compatibility for skins to get the script works well.
I really think that this script is compatible because mentioned in Addons status. Can somebody explain how to setup the Artwork downloaded in a real background manner without having to run it by hand?

You can't!
You used to be able to set it to run every so often but that got changed due to too many api hits on the sites it searches for images. The only way would be to run it as a cron job to call the artwork downloader.

It's a little bit disappointing to do so when everything else in my setup is automatic. I will try this approach or using the Texture Cache Maintenance utility. This tool seems promising and definitively needs to be tested

Add Artwork Downloader as a submenu item. This is how most would do it.
