Disabled:
-Aero
-Unused audio devices
-Excluded all xbmc related folders from my virus software.
Updated nvidia drivers.
Now it seem to worked correct, at least with this evenings movie.

I'm not sure if these settings 'degrade' audio quality but if it does it's certainly a lot more bearable than out of sync a/v
